Patient Registration

To register at the Practice you need to complete a Registration form.  This can be either downloaded using the link below or collected from the Surgery Reception.  

New Patient Registration Form

The registration forms need to be completed in full and returned to the Surgery in person for registration to be completed.  You will need to bring with you a form of photographic ID (driving licence or passport) for proof of identity and a utility bill or bank statement for proof of address.

Every patient at St Katherines Surgery has an allocated named accountable general practitioner who is responsible for the patients overall care at the practice.  You are still free to see any GP that you wish to in the practice but your named accountable GP will take overall responsibility for the co-ordination of your care.  If you wish to know who your accountable GP is please ask at reception.  If you have a preference regarding your named accountable GP we will always try and accommodate this request although this may not always be possible if a particular GP already has an oversubscribed patient list.  

You have the right to express a preference of practitioner when you register with the practice however you will appreciate you may not always be able to see the doctor of your choice at short notice—please tell the receptionist when you register if you have a preferred practitioner.
